New Employee Checklist
- Review the Benefits booklet or on-line enrollment guide
- Log in and make your elections by the date indicated with your enrollment notification
- Print out your enrollment confirmation statement
- Submit your Dependent(s) Verification Form and Documentation
- Look for your ID card to arrive in the mail. Medical/Pharmacy information will be on one ID card.
- Confirm your payroll deductions for benefits elected
- Keep your address up to date by notifying your location’s payroll department
- Call the Preferred Therapy Benefits helpline with any questions at 800-201-7898
